Appointments and communications
Appointment requests, contact-form messages, and "Ask us" assistant conversations submitted through this site are requests only — they are not confirmed bookings and are not monitored continuously. Please do not submit urgent clinical questions or detailed medical history through the website; call the clinic at (708) 808-CIDP for anything time-sensitive or confidential.
AI assistant
The optional "Ask us" assistant is an AI text chat. AI can make mistakes — its answers are general information only, never medical advice or a diagnosis, and it cannot handle emergencies. It does not accept photographs, lab results, or documents: please call the clinic to share anything clinical, and do not send medical records or insurance details through the assistant.
Email sent directly to the Practice's contact address is screened and individually reviewed before it is distributed to staff; automated or unsolicited commercial mail may be held or declined without notice.
Third-party services
Portions of this site rely on third-party platforms — including the electronic health records scheduler, the patient portal, and the optional "Ask us" text assistant. Your use of those services is governed by their own terms and privacy policies, as described in our Privacy Policy.
Acceptable use
